Job Summary:

The primary role of a Care Coordinator is to advocate for clients to make sure they get high-quality care. They perform a range of administrative tasks, such as entering client data, answering phone calls and scheduling client visits. The role involves a significant amount of communication with clients, their family members and various Healthcare Professionals. The Care Coordinator synthesizes information from these people, such as client needs and healthcare goals, in a way that’s useful and helpful to the client and Healthcare Professionals.

Roles and responsibilities will include the following but not limited to :

Ensuring that the staff deliver high-quality and personalized care to client.

Ensuring health and safety compliance.

Liaising with local community organizations and maintaining good relationships with stakeholders.

Ensuring that all activities such as personal care and medication administration meet the required standards.

Preparing reports related to the progress of clients.

Planning and utilizing resources and staying updated with the clinical knowledge to ensure that clients receive the best care.

Collaborate with the care team to assess and evaluate intervention and determine when and where changes should be done.

Develop care plans for patients and restructure them when difficulties arise.

Work in compliance with healthcare and organizational laws and regulations.

Consulting with clients and caregivers, including family members about their health problems.

Read, attend seminars and collaborate with professionals to stay up to date with the developments in the field.

Other related duties as assigned.

Qualifications :

A Bachelor’s degree or certification in a behavioral health field is preferred; in place of a Bachelor’s degree or certification, the candidate must have at least three years working in a relevant professional role and a high school diploma or G.E.D.

Excellent verbal and written communication skills and the ability to prepare comprehensive reports.

A valid driver’s license, verifiable good driving record, and reliable transportation.

Strong interpersonal and communication skills; effectively working with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds.

Passion for helping people and providing personalized care.

Ability to maintain a high level of confidentiality and respect for clients.

In-depth understanding of the organization’s policies and medical regulations.

Demonstrated ability to collaborate and problem-solve.

Proficiency in the Microsoft Office Suite, email systems, and adaptability in learning new software.
