Job Description Job Description POSITION OVERVIEW:
Care Coordinator under supervision of Program Director is the liaison between both the provider and the patient. Schedules appointments for both internal and external services. Conduct follows ups to ensure patient kept appointment and results and/or consultation reports are available for patients follow up visit with the provider.
KEY ESSENTIAL FUNCTIONS:
Facilitates interdisciplinary conversation and planning with team.
Assist with some form completion.
Will assists with direct service to the clients.
Responsible for entering data in the EMR (Electronic Medical Record) system
Conduct outreach for Managed Care health measurements.
Conduct outreach and engagement via phone, electronic methods, and letter patient /collateral/provider to engage clients or strengthen connectivity.
Maintains compliant paperwork to justify service delivery.
Update weekly tracking on care coordinator panel in the EMR system.
Demonstrate the ability to clearly articulate, verbally and in writing, the aims, and goals of the department to potential patients, community members and staff.
Access and respond per agency guidelines to client complaints of grievances
Creating more effective linkages between vulnerable populations and health care system
Providing culturally appropriate health education on topics related to chronic disease prevention, physical activity and nutrition, and cultural competence.
REQUIREMENTS:
HS Diploma required
Bilingual (English and Spanish) preferred
Two years' experience in Care Coordination.
