Community Bridges, Inc. is the largest non-profit provider of substance abuse and behavioral health treatment in Arizona. We currently seek an FACT Peer Support Specialist for our FACT3 team in Phoenix.
The F/ACT Peer Support Specialist (PSS) will work as part of the of the clinical team dedicated to SMI clients. The PSS coordinates services based on daily staffings with other members of the clinical team, provides experience, strength, and hope to individuals, their families, and significant others.
Monitors daily activities and coordinates care with supportive members and providers.
Provides in field outreach and checks to ensure the member is meeting desired treatment goals.
Ensures required face to face interaction with each assigned client is provided at least weekly, and documented accordingly.
Facilitates peer led groups.
Coordinates with family members and team on service delivery methods and options.
Provides in field crisis and other related supportive services.
Works with members to develop a role for themselves outside of their mental illness, while focusing on expected outcomes of increased employment, meaningful community activity, and independent living activity as well as decreased substance us/abuse.
Provides outreach and engagement activities daily, in addition to after hour response while on-call or when member requests assistance.
Other duties as assigned.
Skills/Requirements:
Bachelor’s degree in a field related to behavioral health OR a combination of education, training, and/or experience meeting standards for BHT.
One year of experience working with the SMI population with additional experience in multiple settings and levels of care, including both mental health and substance abuse.
One year of experience in a behavioral health position providing outreach and engagement activities.
Ability to pass criminal background check for jail visitation/clearance.
Valid AZ Driver’s License with 39 month clear MVR
The Peer Support Specialist is an individual who is in recovery from substance use and/or GMH for an appropriate period of time, greater than or equal to 1 consecutive year.
This position requires that the candidate is an experienced Peer Support Specialist.

Qualifications Education Preferred
High School or better.
Experience Required
1 year: Abstinence from Substance abuse, GMH or homelessness.
