Service Coordinator

Assists individuals and families with service coordination in the Indiana PathWays for Aging program.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Assess the need and eligibility of enrolled individuals/clientele through monthly phone contact and quarterly
face to face visits to review effectiveness of service plans, complete assessments, and accurately document all interactions within prescribed time frames. Use independent discretion and judgment in developing intervention strategies and comprehensive plans of
care with clearly stated goals and objectives in conjunction with client, family, or others. Communicate with the individual’s clinical care team to address gaps in care and provides necessary support to
the individual. Act as an advocate to preserve client rights and maintain client confidentiality.
Maintain up-to-date knowledge of services, programs and intervention strategies affecting the Indiana
PathWays for Aging members. Meet and maintains on an annual basis training requirements set forth by the organization and external partners including FSSA and Managed Care Organizations.
Compiles records and prepares reports within prescribed time frames.
Travel locally as needed.
Perform other duties as assigned.

EDUCATION and/or EXPERIENCE
Bachelor’s or Master’s Degree in social work, psychology, counseling, gerontology, nursing, or health & human services, or have a Bachelor’s Degree in any field with at least 2 years of full-time direct service experience with older adults or persons with disabilities, or have an Associate’s Degree in any field with a minimum of four (4) years full-time direct service experience with older adults or persons with disabilities, or be a registered nurse of licensed practical nurse and have one year of experience service the PathWays program or like populations.
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
