Pay Rate: $22.00/hour - $24/hour (based on experience)
Schedule: M-F 8 AM to 5:300 PM

Job Duties:
Develop, assess and adjust, as necessary, the care plan and promote desired outcome

Coordinate services between Primary Care Physician (PCP), specialists, medical providers, etc.
Develop plan of care based upon assessment with specific objectives, goals and interventions
Facilitate member access to community based services
Monitor referrals made to community based organizations, medical care and other services
Enter and maintain assessments, authorizations, and pertinent clinical information into various medical management systems
Education/Experience: Required Bachelor's degree in Human Services.
2+ years of clinical nursing experience in a clinical, acute care, or community setting. Knowledge of healthcare and managed care preferred.
Must be able read, write, and speak in both Spanish and English.

Certifications/Licenses: Current and valid CPR certification.
