As a Care Coordination Specialist you will :
· Serves as a patient advocate and assists in identification and improvement of departmental outcomes
· Talent to be able to establish and maintain positive working relationships with patients, family, caregivers, physicians, and clinical and non-clinical staff
· Support Care Managers
A Day in the Life:
· Manage internal and external customer relationships to ensure that all customer service needs are being met, including assisting offices in understanding their performance
· Ability to maintain effective and professional relationships with patient, physicians and their practices, and other members of the care team
· Handle a high workload and meet deadlines with a high level of accuracy
· Determine Quality Measures in Value Based Care Programs
What Success Looks Like in this Role:
· Must be compassionate, professional, have exceptional communication/telephonic skills, detailed-oriented, and capable of handling a very fast-paced environment
· Ability to facilitate multiple components of care to include but not limited to scheduling, gaps in care closure, and ensuring all collected data is entered into the care management module accurately
· A personal commitment to a person-centered approach, along with strong relationship-building, communication, and active listening skills required
· Serves as a patient advocate
Requirements
We are excited about you if you have these things:
· High School diploma required, some college preferred
· 4+ years of health care customer service or call center experience preferred
· Medical Assistant with call center experience desired
· Medical Terminology required
· Knowledge of population health a plus
· Bilingual a plus
