Care Coordinator

The Always Best Care's Care Coordinator (CC) position is responsible for coordinating the care delivery for Always Best Care homecare/non-medical clients. The CC is responsible for teaching, coaching, and supporting the Direct Care Worker (DCW) to ensure the client's specific care delivery is provided. The goal of this position is to enhance the client's quality of care by coordinating the care delivery process in a seamless, safe, and effective manner. The CC will maintain and oversee client's care coordination by developing relationships with internal and external customers to better meet and anticipate client's needs. The coordination of care begins at the Start of Care (SOC) until the discharge process from Always Best Care's services is completed. The CC will monitor the client for the duration of their care with Always Best Care by completing admission visits, follow-up visits and quality management calls to ensure the client's care delivery and satisfaction is maintained. The CC will work a flexible schedule which is 8:00 am - 4:30 pm and alternating weekends (as needed). This position reports directly to the Care Coordinator Supervisor.
Job Functions :
Completes the Start of Care Process by initiating the admission process to service all care level clients including clients receiving Specialized Care Services/Activities
Develops and supports the client's care plan accordingly.
Ensures all required documentation is complete, legible and timely.
Educates Direct Care Workers (DCW) by hands-on return demonstration to ensure competency, classroom training and video training when applicable.
Maintains client's notes in EHR timely and accurate.
Utilizes all lines of communication EHR, emails, phone, fax, and verbal.
Reports to the RN when there is a change in the client's condition.
Completes a Home safety check list and follow up accordingly.
Communicates with other Healthcare providers ex. Case workers.
Works with vendors, referral sources and Admin team to ensure the best client outcomes.
Manages time and attention to details.
Adheres to Always Best Care's Policies & Procedures.
Must attend Annual In-Servicesand any other required educational meetings/webinars. that will enhance job performance.
Provides on-going in-home training/competencies for the DCWs.
Presents new hire/ annual orientations as needed.
Provides recommendations for DCW training needs.
Communicates with RN for additional training that the CCP may need to adequately perform their job.
Abides by all HIPAA and associated patient confidentiality requirements
Maintain a current CPR/ for best practice
Communicates departmentally
Promotes quality services to internal and external customers
Essential Qualifications
High school graduate or GED
Must possess a reliable, personal vehicle along with maintain and provide a valid PA driver's license, vehicle registration, & auto insurance
At least two (2) years of experience working with seniors in a home setting
CNA License is preferred
Mechanical lift and assistive device experience preferred
English proficiency and excellent communication skills
Ability to work independently in the field
Ability to work with different personality types in a professional manner
Proof of a clear 2-step TB/PPD test taken within the last year showing clear results
Must be able to pass background check
Child Abuse Clearance is required
Proficient in Microsoft office applications
Ability to navigate to service areas
