The
Admissions RN
conducts new patient assessments and admissions, to determine that the patient meets program eligibility requirements, as well as existing patient re-certifications. This position provides flexibility to visit patient homes and do follow up and scheduling from your home!
Schedule:
Monday – Friday
Times:
8 am to 5 pm
***Payrate : $80,000 to 90,000 per year***
Responsibilities:
The Admissions RN conducts home visits to new patients; obtaining a medical history, particularly as it relates to the present condition, from the client and/or family member(s).
Productivity: Conduct min. of 12 - 20 admissions per week. Min case load is 35 visits per week., including
recertification
.
Conducts a physical examination of the client, including vital signs, physical assessment, mental status, appetite and type of diet, etc.
Evaluate the client, family member(s) and home environment/ situation to determine what health teaching will be required, assistance from the family that may be available, and how to best create a care plan for client.
Make recommendations to leadership for candidate admissions.
In collaboration with other clinicians, the Admissions RN designs a plan of care for qualified patients.
